Before this existed there was almost nothing for erectile difficulty that did not involve a device or an injection.
Sildenafil changed that, and the molecule has not needed improving — the newer options are variations on the same idea.
Without prescription, plain packaging. Nitrates are an absolute no.
It was being investigated as a treatment for angina. The reasoning made sense: relaxing blood vessels should improve blood supply to the heart.
It did not work well enough for that. What came out of the trials instead was a consistent and unexpected effect that the participants were notably reluctant to return unused tablets over.
The same mechanism, in a different place. The chemistry was never the problem — the target organ was.
What the mechanism actually is
An erection is hydraulics, triggered by the nervous system.
Arousal releases nitric oxide into the erectile tissue. That sets off a messenger molecule which relaxes the smooth muscle in the small arteries there.
Relaxed arteries let blood in. The filling tissue presses on the veins that would drain it, so the blood stays. Hence rigidity.
An enzyme called PDE5 breaks down the messenger, which is how an erection ends normaly.
Sildenafil blocks PDE5. The messenger survives longer, so the process holds.
Note what that means: it does not create arousal and it does not start anything. If there is no nitric oxide released, there is no messenger to preserve. It amplifies what is already happening.
The fatty meal problem
This one is specific to sildenafil and it is the most common reason it disappoints.
A large, fatty meal substantially delays absorption. Take it after a heavy dinner and the onset can be pushed back by an hour or more.
People conclude the dose is too low when the actual problem was the meal.
Taken on a relatively empty stomach, it works within about thirty minutes to an hour, with a window of several hours.
Taking it
Around an hour beforehand.
Avoid a heavy or fatty meal before it.
Once in twenty-four hours.
Keep alcohol modest.
No grapefruit juice.
Arousal is required.
Give it several attempts before deciding it does not suit. First-time nerves work directly against the thing you are trying to achieve, and plenty of people write off a dose that would have been fine on the third go.
Nitrates: the one hard stop
This is the single line on the page with no room for interpretation.
Glyceryl trinitrate, isosorbide mononitrate, isosorbide dinitrate, nicorandil — anything taken for angina. Including the spray carried in a pocket for emergencies.
Together they can drop blood pressure to a dangerous level. Not mild dizziness. Circulatory collapse.
The same applies to amyl nitrite used recreationally, sold as poppers. That combination reaches emergency departments more often than anyone admits.
If you take anything in that group, this is out. No exceptions, no leaving a gap, no trying half a tablet.
Who it is not for
Recent heart attack or stroke
Unstable angina
Severe heart failure
Uncontrolled high or very low blood pressure
Severe liver disease
Retinitis pigmentosa
Previous sudden vision loss due to optic nerve blood supply problems
Alpha blockers for prostate enlargement need care, since the blood pressure effects add together.
Possible side effects
Headache
Facial flushing and a sense of warmth
Blocked nose
Indigestion
A blue tinge to vision or sensitivity to light
Dizziness
The visual effect comes from partial action on a related enzyme in the retina. It is temporary and harmless, though it surprises people who have not been told about it.
Two things that need help immediately
An erection lasting more than four hours. This is not something to wait out. After several hours the tissue begins to sustain permanent damage, and the window for treating it closes.
Sudden loss of vision or hearing, in one or both sides. Stop taking it and get seen urgently.
Both are rare. Both are measured in hours rather than days.
It can be an early warning
This part matters more than the tablet does.
The arteries supplying erectile tissue are narrow. Narrowing shows up there before it shows up in the wider coronary arteries — sometimes by several years.
So difficulty that developed gradually over months can be the first visible sign of vascular disease. That makes it an opportunity rather than only a problem: blood pressure, blood sugar and cholesterol are worth checking.
The picture is different if it came on suddenly, happens only in certain situations, and morning erections are still there. That pattern usually points to stress, exhaustion, a relationship, or a medicine you have started.
What helps independently of any tablet
Stopping smoking — blood vessels respond faster than most people expect
Regular aerobic exercise
Reducing excess weight
Less alcohol
Sleep, consistently
Pelvic floor training, which has better evidence than its reputation suggests
Reviewing your own medicines
That last point gets skipped. Several blood pressure medicines, antidepressants and prostate treatments contribute. Changing one is sometimes simpler than adding another tablet.
Ordering without prescription: how it works
Keeping some at home is the main reason people order online rather than asking at a counter.
Pick the strength and the pack size, put in a delivery address, confirm. Couple of minutes, any time of day, no queue and no conversation across a counter.
It goes out in plain packaging. No pharmacy name on the outside, nothing that hints at what is inside. How long delivery takes depends on where the parcel is headed.
On price it comes down to strength and pack size. Larger packs generally work out better per unit.
Delivery goes anywhere in the UK, in plain outer packaging with no logo and no product name on it. How long it takes depends on where it is heading and which service you picked. Prices are in pounds.
