Dulcolax: when nothing else has worked

Constipation is one of those complaints people put up with far longer than they need to. Bisacodyl stimulates the nerves in the bowel wall directly, which gets things moving when gentler approaches have not. Without prescription, plain packaging. Swallow whole, and do not take it with milk.Where does it belong in the order of things?
A stimulant laxative is not the first thing to reach for. It is what you use when the simpler measures have not worked.
- More fluid — without it, extra fibre makes things worse
- Fibre: vegetables, wholegrains, linseed, prunes
- Movement, even just walking
- A consistent time and no rushing in the bathroom
- Not ignoring the urge when it comes
Worth checking too: a lot of constipation is caused by medication. Iron supplements, certain painkillers, some blood pressure tablets and some antidepressants are common culprits. Addressing that beats treating the symptom.
Then, if things still have not moved, a stimulant.
How it gets things going
Bisacodyl works on the nerve network in the bowel wall.
It stimulates those nerves, increasing the wave-like contractions that move contents along. It also reduces how much water is reabsorbed from the stool, so it stays softer.
Faster transit plus more water retained. Both push in the same direction.
It generaly takes somewhere between six and twelve hours, which is why the evening is the standard time to take it.
The coating is doing a job
This is the detail that trips people up, and it has real consequences.
The tablet is coated so that it passes through the stomach intact and only releases its contents in the intestine, which is where it needs to act.
If that coating dissolves early, the drug is released in the stomach. The result is cramping and nausea, and much less effect where it was supposed to work.
Two things dissolve the coating prematurely:
- Crushing or chewing the tablet. Swallow it whole, always.
- Milk or antacids. Both reduce stomach acidity, which is what the coating depends on to stay intact.
Leave at least an hour between the tablet and any milk, dairy or antacid. It is a small thing that changes the outcome entirely.
Not for every day
Short-term use is what this is designed for.
Used continuously over long periods, the bowel can become less responsive and you end up needing it to function at all. That is a difficult pattern to unwind.
Prolonged use also depletes potassium, which causes weakness and cramps — and, awkwardly, sluggish bowel function, which makes the constipation worse.
For ongoing constipation, bulk-forming or osmotic laxatives are the ones intended for long-term use. A stimulant is for getting past a blockage, not for running the system.
Taking it
- In the evening, swallowed whole with water.
- Expect an effect the following morning.
- No milk or antacid within an hour.
- Short course only.
- Keep fluid intake up throughout.
Possible side effects
- Abdominal cramping
- Nausea
- Diarrhoea if the dose is too high
- Dizziness, usually from fluid loss
- Low potassium with prolonged use
Cramping is the most common complaint and is usually either a dose that is too high or a coating that dissolved early.
When constipation is not just constipation
- Severe abdominal pain
- Vomiting with no bowel movement at all
- Blood in the stool, or black stools
- Unexplained weight loss
- A change in bowel habit lasting weeks, especially over the age of forty
- Alternating constipation and diarrhoea
- Persistent abdominal swelling
A laxative treats a symptom. Those points need a cause identified, not a stronger laxative.

Constipation does not announce itself in advance, which is why this is usually bought before it is needed.
Pick the strength and the pack size, put in a delivery address, confirm. Couple of minutes, any time of day, no queue and no conversation across a counter.
