How it works
Arousal releases nitric oxide into the erectile tissue. That produces a messenger which relaxes the smooth muscle around the small arteries there.
Blood flows in. The filling tissue compresses the veins that would drain it, so the blood stays. That is the mechanics.
PDE5 is the enzyme that breaks that messenger down and ends it. Tadalafil blocks PDE5.
Which means arousal is still essential. Nothing is being switched on; something is being prevented from switching off. A long window does not mean a permanent erection — nothing happens until there is arousal.
Taking it
Around thirty minutes to an hour beforehand.
With or without food — a meal matters little here.
Once in twenty-four hours at most.
Keep alcohol modest.
No grapefruit juice.
The long duration makes the once-daily limit more important, not less. The previous dose is still substantially present.
It also helps prostate symptoms
Worth knowing, because it is a genuine dual benefit.
Tadalafil relaxes smooth muscle in the prostate and bladder neck as well as in erectile tissue. For men with an enlarged prostate that can ease the urinary symptoms — the weak stream, the hesitancy, getting up at night.
So for someone dealing with both, one medicine addresses both. That is one reason the low-dose daily version exists.
Nitrates: the one hard stop
This is the single line on the page with no room for interpretation.
Glyceryl trinitrate, isosorbide mononitrate, isosorbide dinitrate, nicorandil — anything taken for angina, including the spray carried for emergencies.
Together they can drop blood pressure to a dangerous level. Not mild dizziness. Circulatory collapse.
The same applies to amyl nitrite used recreationally, sold as poppers.
And with tadalafil there is an extra consideration: it stays in the body far longer than the others. A gap of a day or two is not enough. If you take nitrates in any form, this is out entirely.
Who should not take it
Recent heart attack or stroke
Unstable angina, or chest pain on light exertion
Severe heart failure
Uncontrolled high blood pressure, or markedly low blood pressure
Severe liver or kidney disease
Retinitis pigmentosa
Previous sudden vision loss from reduced optic nerve blood supply
Alpha blockers for an enlarged prostate lower blood pressure as well, and the effects add. That pairing needs arranging deliberately.
The back ache, and where it comes from
Back and muscle ache turns up more with tadalafil than with the alternatives, and there is a reason.
These drugs block PDE5, but none of them does so perfectly. Tadalafil also has some effect on a related enzyme, PDE11, found in skeletal muscle.
That is the leading explanation for the aching lower back and thighs some men notice, typically starting a day or so after taking it.
It is harmless and it passes. An ordinary painkiller handles it. For some people a lower strength avoids it while keeping most of the effect.

Flushing
Nasal congestion
Muscle ache
Dizziness
Visual effects are less common with tadalafil than with sildenafil, because it has less action on the retinal enzyme.
Two things that need help immediately
An erection lasting more than four hours. Not something to wait out — after several hours the tissue begins to sustain permanent damage.
Sudden loss of vision or hearing, one side or both. Stop and get seen urgently.
Both rare. Both measured in hours, not days.
What it may be telling you
The arteries supplying erectile tissue are narrow, and narrowing appears there before it shows in the coronary arteries — occasionaly by several years.
Difficulty that developed gradually is therefore worth taking as a prompt to check blood pressure, blood sugar and cholesterol.
A sudden onset, confined to particular situations, with morning erections intact, usualy points to stress, exhaustion, the relationship, or a medicine recently started.
