Practical rather than pharmacological.
Two seperate tablets means two timings to coordinate, two things to remember, and two things to keep track of. For something taken before an occasion, that friction matters.
A combination removes it. One tablet, one timing.
The trade-off is less flexibility. You cannot adjust one component independently, which is a genuine limitation if one half needs more and the other less.
Sildenafil acts locally. Arousal releases nitric oxide in the erectile tissue, producing a messenger that relaxes the smooth muscle of the small arteries. Blood flows in, the engorged tissue compresses the draining veins, and it stays. PDE5 would normaly destroy that messenger and end the erection — sildenafil blocks PDE5.
Dapoxetine acts centrally. Ejaculation is governed by a spinal reflex that serotonin dampens. Dapoxetine increases serotonin availability at the relevant synapses, raising the threshold required to set the reflex off.
Different sites, different systems, no interference between them.
Specific to this combination, and the most common avoidable problem with it.
Sildenafil absorption is noticeably delayed by a large fatty meal. In a combination tablet that shifts the erection component later, which means the window where both halves are working properly moves.
So if dinner is part of the evening, keeping it moderate matters more here than with a tadalafil-based combination.
Give it several occasions. The first attempt carries expectation and self-monitoring, which work against both components.
It can lower blood pressure, particularly on standing, and occasionally causes fainting.
Stand up gradually. If you feel lightheaded, nauseated or clammy, lie down with your legs raised rather than pushing through.
Alcohol increases this substantially, and both components lower blood pressure — so the combination is more sensitive to it than either alone.
Glyceryl trinitrate, isosorbide mononitrate, isosorbide dinitrate, nicorandil — anything for angina, including the emergency spray. Also amyl nitrite sold as poppers.
Together the drop in blood pressure can be life-threatening. Not dizziness — circulatory collapse.
If you take anything in that group, this is out entirely.
Serotonin excess shows as agitation, confusion, muscle twitching, fever, racing heart and diarrhoea. Urgent.
Alpha blockers for prostate enlargement add to the blood pressure effect, which matters more here since both components lower it.
An erection lasting more than four hours. After several hours the tissue begins to sustain permanent damage.
Sudden loss of vision or hearing. Stop and get seen urgently.
A combination tablet is only convenient if it is already to hand.
Pick the strength and the pack size, put in a delivery address, confirm. Couple of minutes, any time of day, no queue and no conversation across a counter.
It goes out in plain packaging. No pharmacy name on the outside, nothing that hints at what is inside. How long delivery takes depends on where the parcel is headed.
On price it comes down to strength and pack size. Larger packs generally work out better per unit.
