Priligy: taken before, not every day

When does it count as a problem?

The answer is less clinical than people expect.

The definition involves consistently rapid ejaculation, little sense of control over it, and distress resulting from it. That third element is the one that decides.

It is not a stopwatch question. It is whether it bothers you or your relationship. Some couples do not experience it as a problem at all; for others it becomes the dominant issue.

Also worth saying: expectations about what is normal are heavily distorted by what is depicted in pornography. Reality is different, and simply knowing that removes a surprising amount of pressure.

Two patterns, two sets of causes

Lifelong. Present from the first sexual experiences. Usually reflects how the reflex is set rather than any illness.

Acquired. Developed after a period when things were fine. This one more often has a findable cause.

Things worth checking in the acquired pattern: an overactive thyroid, prostate inflammation, erectile difficulty, a new medication, significant stress, or tension in the relationship.

Erectile difficulty is a particulary common and overlooked cause. A man worried the erection will not hold hurries without deciding to.

How it works

Ejaculation is governed by a reflex in the spinal cord. Serotonin acts as a brake on that reflex.

Dapoxetine blocks serotonin reuptake, so more remains available at the relevant synapses. The threshold needed to trigger the reflex rises, and a stronger stimulus is required.

Chemically it belongs to the same family as some antidepressants, with one crucial difference: it acts very quickly and clears from the body just as fast.

That is precisely why it is taken on demand rather than daily.

When to take it

• About one to three hours beforehand.
• Swallow whole with a full glass of water.
• With or without food.
• Once in twenty-four hours at most.
• No alcohol.

The glass of water is not decorative. It reduces the risk of fainting, which is the main concern with this drug.

And the first tablet often does not represent what it can do, in either effect or tolerance. Several occasions give something you can actually judge.

Fainting: the thing to know

It can lower blood pressure, particularly on standing, and occasionally causes fainting.

So stand up gradually rather than abruptly. If you feel dizzy, nauseated or clammy, lie down immediately with your legs raised rather than trying to walk it off.

Do not drive or operate machinery until you know how you react.

Alcohol increases this risk substantially. That is not caution in small print — it is the main factor in reported incidents.

Serotonin interactions

• SSRI and SNRI antidepressants
• Tricyclic antidepressants
• MAO inhibitors
• Tramadol and some other painkillers
• Triptans for migraine
• St John's wort, including as a herbal product
• Linezolid, lithium

Signs of serotonin excess: agitation, confusion, muscle twitching, fever, racing heart, diarrhoea. That needs urgent help.

Long gaps are required after stopping any of those, often weeks. St John's wort is missed constantly because people do not count it as medication.

Who should not take it

• Heart disease, heart failure, rhythm problems or valve disease
• Previous heart attack or stroke
• History of fainting
• Low blood pressure
• Severe liver disease
• Mania or bipolar disorder
• Severe depression
• Under eighteen

In bipolar disorder, serotonin-acting drugs can trigger mania. That is a real risk, not theoretical.

Possible side effects

• Nausea, the most common of all
• Dizziness and headache
• Diarrhoea
• Insomnia
• Dry mouth
• Sweating
• Anxiety, irritability
• Visual disturbance

Report mood changes, thoughts of self-harm or marked anxiety — that applies across the whole serotonin-acting class.

What works alongside the tablet

• The stop-start technique, learned together
• Pelvic floor training, genuinely effective and widely ignored in men
• Condoms that reduce sensation
• A longer build-up that takes the focus off penetration
• Talking about it, which cuts the pressure more than anything else
• Sex therapy, particularly where the problem started suddenly

A tablet to break the cycle plus technique in the background works better for most men than either alone — largely because the pressure drops, and pressure was half the problem.

Ordering without prescription: how it works

On-demand use means the tablet needs to be in the house before the occasion, not ordered for it.

Pick the strength and the pack size, put in a delivery address, confirm. Couple of minutes, any time of day, no queue and no conversation across a counter.

It goes out in plain packaging. No pharmacy name on the outside, nothing that hints at what is inside. How long delivery takes depends on where the parcel is headed.

On price it comes down to strength and pack size. Larger packs generaly work out better per unit.

If something arrives damaged anywhere in the UK, send the order number and a photograph the same day. We replace free of charge or refund in pounds. Do not use medication from a damaged blister.
