Vardenafil with dapoxetine: both problems, one tablet

Free bonus

When both things are happening together

The two most common sexual difficulties in men often arrive as a pair, and one usually drives the other. This combination pairs vardenafil, which handles the mechanics, with dapoxetine, which raises the ejaculation threshold. Without prescription, plain packaging. Two sets of interactions to check, not one.

Why do they travel together?

It looks like bad luck. It is more often a loop.

A man unsure whether the erection will hold hurries, usually without deciding to. Hurrying shortens the encounter. A shorter encounter raises the pressure next time. More pressure makes the erection less reliable.

Treating one side alone often disappoints, because the loop keeps turning on the other side.

Worth being honest with yourself though: if timing alone is the issue and the erection is fine, you do not need the first component.

What each half does

Vardenafil works in the erectile tissue. Arousal releases nitric oxide, producing a messenger that relaxes the small arteries. PDE5 destroys it; vardenafil blocks PDE5 so it persists.

It is the more selective of the PDE5 inhibitors, which means fewer visual side effects than sildenafil.

Dapoxetine works centrally. Ejaculation is governed by a spinal reflex that serotonin dampens. Dapoxetine increases serotonin availability at the relevant synapses, raising the threshold required to trigger it.

Two different systems, two different sites, no interference.

Taking it

One to three hours beforehand.
Swallow whole with a full glass of water.
Once in twenty-four hours.
No alcohol.
Arousal required for the erection component.
A heavy fatty meal slows absorption.

Several attempts before judging it. The first occasion carries the expectation that works against the result.

Fainting: the main dapoxetine concern

It can lower blood pressure, particularly on standing, and occasionaly causes fainting.

So: swallow it with a full glass of water, stand up gradually, and if you feel lightheaded, nauseated or clammy, lie down with your legs raised rather than trying to push through standing.

Alcohol increases this risk substantialy. That is not small print — it is the main factor in reported incidents.

Serotonin interactions

Dapoxetine acts on serotonin, and combining it with other serotonin-acting drugs can cause a dangerous excess.

SSRI and SNRI antidepressants
Tricyclic antidepressants
MAO inhibitors
Tramadol and some other painkillers
Triptans for migraine
St John's wort, including as a herbal product
Linezolid, lithium

Signs of serotonin excess: agitation, confusion, muscle twitching, fever, racing heart, diarrhoea. That needs urgent help.

Long gaps are required after stopping any of those, often weeks. St John's wort gets overlooked because people do not think of it as medication.

Nitrates: the one hard stop

This is the single line on the page with no room for interpretation.

Glyceryl trinitrate, isosorbide mononitrate, isosorbide dinitrate, nicorandil — anything taken for angina, including the spray carried for emergencies.

Together they can drop blood pressure to a dangerous level. Not mild dizziness. Circulatory collapse.

The same applies to amyl nitrite used recreationally, sold as poppers. That combination reaches emergency departments more often than anyone admits.

If you take anything in that group, this is out. No exceptions, no gap, no half tablet.

The heart rhythm point, specific to vardenafil

This distinguishes vardenafil from the others in the class and it is worth knowing.

It has a modest effect on the electrical recovery time of the heart — what is measured as the QT interval. On its own that is not usually a problem.

It becomes one in combination with other medicines that do the same thing, because the effects add.

Certain heart rhythm medicines, including amiodarone, sotalol and quinidine
Some antipsychotics
Certain antibiotics, particularly some macrolides and quinolones
Some antifungals
A few antidepressants

A known long QT interval, or a family history of sudden unexplained death, means this molecule is not the right choice. One of the alternatives is.

Who should not take it

Significant heart disease, heart failure, rhythm problems or valve disease
Previous heart attack or stroke
History of fainting
Low blood pressure
Severe liver or kidney disease
Known long QT interval
Bipolar disorder or mania
Severe depression
Retinitis pigmentosa
Under eighteen

In bipolar disorder, serotonin-acting drugs can trigger a manic episode. That is a real risk.

Possible side effects

Nausea, the most common from the dapoxetine side
Headache, dizziness
Flushing and blocked nose from the vardenafil side
Diarrhoea
Insomnia
Dry mouth, sweating
Visual disturbance

Report any change in mood, unusual agitation or thoughts of self-harm. That applies across the serotonin-acting class.

Two things that need help immediately

An erection lasting more than four hours. Not something to wait out — after several hours the tissue begins to sustain permanent damage.

Sudden loss of vision or hearing, one side or both. Stop and get seen urgently.

Both rare. Both measured in hours.

What helps alongside the tablet

Stop-start technique, practised as a couple
Pelvic floor training, consistently underrated
Condoms that reduce sensation
A longer build-up that takes the focus off penetration
Talking about it, which does more to reduce pressure than anything else

A tablet to break the cycle, plus technique in the background, works better for most men than either alone. Mostly because the pressure drops, and pressure was half of it.

Ordering without prescription: how it works

One tablet instead of two is most of the appeal, and that only works with a supply in hand.

Pick the strength and the pack size, put in a delivery address, confirm. Couple of minutes, any time of day, no queue and no conversation across a counter.

It goes out in plain packaging. No pharmacy name on the outside, nothing that hints at what is inside. How long delivery takes depends on where the parcel is headed.

On price it comes down to strength and pack size. Larger packs generally work out better per unit.

Parcels leave on working days, so UK public holidays and weekends push everything back. If it matters to a date, order with a few days to spare rather than none. Nobody can promise a calendar day.
