A nail is a dense plate of keratin. It is designed to be a barrier, and it is very good at it.
Creams and lacquers barely penetrate it. The fungus lives in the nail bed underneath, where topical treatment essentially does not reach.
Tablets take a different route entirely. The drug travels through the bloodstream and is deposited into newly forming nail tissue from below.
That is also used for extensive ringworm or persistent jock itch that has not cleared with creams.
Terbinafine belongs to a class called allylamines, and it is specifically potent against dermatophytes — the filamentous fungi responsible for the large majority of nail and skin infections.
Against yeasts it is considerably weaker.
Which is a good reason to identify the organism before committing to months of treatment. If it turns out to be a yeast, a different drug is the right answer.
The mechanism is worth a paragraph because it explains the strength of the effect.
Fungi need ergosterol for their cell membranes. Terbinafine blocks an enzyme very early in the pathway that produces it.
That has two consequences simultaneously. There is no ergosterol at the end, so the membrane destabilises. And the raw material backs up behind the blockage — and that accumulated compound is itself toxic to the fungal cell.
So it kills fungi rather than merely stopping them growing. That difference is why a course can actually cure rather than suppress.
This is where most courses fail, and it is purely a matter of expectation.
The drug deposits into newly formed nail. The already-infected portion will never improve — it has to grow out.
A fingernail takes several months to replace itself. A toenail takes considerably longer.
So treatment lasts months and, for most of that, nothing visible changes.
What is actually happening: healthy nail is forming at the base while the diseased part slowly migrates outward. Photograph the nail monthly — the progress is obvious in pictures and invisible day to day.
Do not stop because nothing seems to be happening. Something is.
The most important safety aspect of this treatment.
The drug is processed by the liver and taken for months. Liver function is checked before starting and during the course.
Stop and get checked for: yellowing of the skin or eyes, dark urine, pale stools, persistent nausea, loss of appetite, unusual fatigue, or pain under the right ribs.
Rare. But worth knowing with a treatment this long.
An unexpected side effect that unsettles people who have not been warned.
Some people find their sense of taste changes or temporarily disappears. Food tastes bland or metallic.
It usually begins after some weeks and resolves after stopping — though sometimes only after further weeks or months.
If you are losing weight because eating has stopped being worthwhile, that is worth reporting.
A widespread rash with blistering or involvement of the mouth and eyes means stopping immediately.
Treating the skin and ignoring the footwear is why athlete's foot keeps coming back.
Spores survive in shoes for months. Treat the feet, put the same shoes on, and you reinfect yourself.
It reads like a lot of effort. It is the part that decides whether this takes two weeks or two years.
Do not share nail tools, and disinfect them after use.
Spores persist on metal surprisingly well. It is a common route of transmission within a household, and the reason fungal nail infections can circulate between family members for years.
