For straightforward vaginal thrush a single capsule is the usual approach. Other sites need longer courses.

Taking the wall apart

Fungal cells need a substance called ergosterol to keep their outer membrane intact — much as human cells rely on cholesterol.

Fluconazole blocks the enzyme that produces it.

Without ergosterol the membrane becomes leaky. The cell loses its contents and stops being able to grow or divide.

Human cells do not carry that enzyme in the same form, which is what keeps the drug reasonably selective.

Why does a capsule beat a cream for some people?

Creams and pessaries work, and for a one-off they are perfectly fine.

But a swallowed capsule reaches the tissue through the bloodstream, which means it gets everywhere the infection is rather than only where you managed to apply it. No mess, nothing to insert, and nothing that weakens latex.

That last point is practical: antifungal creams can compromise condoms, and a capsule does not.

When thrush keeps coming back

A recurring infection is a signal, not just bad luck. Treating it again and again without asking why is a long and frustrating road.

A course of antibiotics that wiped out the competing bacteria
Poorly controlled diabetes — sugar in the tissue is fuel
Hormonal shifts, including pregnancy and some contraception
A weakened immune system
Reinfection from an untreated partner
Constant moisture and non-breathable fabric
Over-washing with harsh products, which strips the protective flora

That last one is counterintuitive and very common. Aggressive hygiene makes thrush more likely, not less.

Four or more episodes in a year is worth investigating properly rather than treating again.

Cotton underwear, nothing tight
Plain water for washing; skip scented products entirely
Change out of wet swimwear or gym kit promptly
No douching, ever — it strips the defences that keep yeast in check
If you have diabetes, blood sugar control is part of the treatment

Interactions worth checking

Fluconazole blocks liver enzymes that process a long list of other medicines, so levels of those can rise.

Warfarin — bleeding risk increases
Certain statins — muscle problems become more likely
Some heart rhythm medicines
Phenytoin
Certain diabetes tablets, with a risk of low blood sugar
Some sedatives

For a single capsule this is usually a small matter. For longer courses it needs proper checking, particulary with warfarin or statins.

Stop and get help for a spreading rash with blistering, yellowing of the skin or eyes, or dark urine.

When it is not thrush at all

If nothing improves after treatment, the diagnosis may be wrong. Bacterial vaginosis, trichomonas and simple irritation from a product all cause similar symptoms and none of them respond to an antifungal.

A swab answers it in one step and saves weeks of guessing.
