The flip side: if weight gain is a concern, that is a real consideration. It can be substantial, and the increased appetite tends to persist.
Taking it
Once daily, in the evening, before bed.
With or without food.
Orodispersible tablets dissolve on the tongue — do not push them through the blister.
Expect drowsiness in the first days.
Taking it at night turns the sedation from a problem into the point.
Several weeks before you can judge it
This is the single biggest reason people stop too early.
The level in your blood changes within hours. The effect takes weeks, because what actually helps is a slow rebalancing of the networks involved — not the concentration itself.

Rarely, mirtazapine affects the production of white blood cells.
Which means a sore throat, mouth ulcers, fever or flu-like illness during treatment should be reported rather than assumed to be an ordinary infection. A blood count settles it quickly.
Uncommon, but it is the specific thing worth knowing about this medicine.

Notably, sexual side effects and nausea are much less common than with the SSRIs — a direct consequence of the receptors it blocks.
Alcohol and sedatives
The sedation adds on top of anything else sedating. Alcohol, sleeping tablets, strong painkillers and some antihistamines all compound it.
