Most asthma treatment works on inflammation broadly, or on relaxing airway muscle. This one picks a specific target.
When the immune system is provoked, it releases a family of signalling molecules called leukotrienes. They are powerful stuff — far more potent at tightening airways than histamine.
They narrow the bronchial tubes, pull inflammatory cells into the lining, and drive mucus production.
Montelukast blocks the receptor those molecules act on. The signal is still produced, it just has nowhere to land.

The exercise point deserves emphasis. Leukotrienes are heavily involved in the airway narrowing that follows exertion, and this is one of the better options for people whose main problem is a chest that closes up twenty minutes into a run.
It is not a rescue medicine
Important, because the distinction gets blurred.
This does nothing during an attack. It does not relax tight airway muscle on demand, and it takes hours to reach useful levels.
If you have an attack, your reliever inhaler is the answer. Keep it with you regardless of how well the tablets are working.
Why evening
Asthma symptoms are typically worst in the early hours of the morning. Lung function dips overnight, and that is when people wake up coughing.
An evening dose puts the peak blood level roughly where it is needed.

For exercise specifically, a dose a couple of hours beforehand is sometimes used instead — but not in addition to a daily dose.

Most people take montelukast without any trouble at all. But this needs stating plainly rather than buried.
A minority experience changes in mood or behaviour. Vivid dreams and nightmares, trouble sleeping, irritability, anxiety, low mood, restlessness, and in rare cases thoughts of self-harm.
In children it can show up as unusual agitation, nightmares or a change in behaviour that parents struggle to explain.
It can start within days or after months. If anything like that appears, stop and get advice — do not wait to see whether it passes. It generally resolves after stopping.
This is not a reason to avoid the medicine. It is a reason to know what to watch for, because it is easy to attribute to something else entirely.

Do not stop your inhaler on your own
If you feel well on the tablet, the temptation is to drop the preventer inhaler. That is how people end up in trouble.
Inhaled steroids treat the underlying inflammation in the airway lining. Montelukast works on one pathway. For most people they are not interchangeable.
Any change to a preventer needs reviewing, not deciding unilaterally on a good week.
When control is slipping
Count how often you reach for the reliever. Several times a week, or waking at night with symptoms, means the current plan is not holding.
An attack that does not respond to the reliever is an emergency. Signs: unable to speak a full sentence, lips turning blue, sitting forward propped on the arms, rising panic.
