Worth knowing what it does not cover: roundworms, threadworms and hookworms are a completely different group and need a different drug entirely. Treating the wrong group gets you nowhere.
The mechanism is unusually fast and quite specific.
Praziquantel makes the worm's outer covering suddenly permeable to calcium. Calcium floods in, and the muscles go into sustained contraction — a paralysis the parasite cannot release.
A fluke clamped inside a blood vessel, or a tapeworm hooked into the gut wall, simply lets go.
At the same time the outer covering starts to break down, exposing surfaces the immune system normally never sees. The body then clears what is left.
All of this begins within minutes of the drug reaching the parasite, which is why the course can be so short.
Food is not optional advice here. The drug absorbs noticeably better with a meal, and that affects whether it works.
This throws people, so it is worth explaining before it happens.
When a mass of parasites dies at once, the material released sets off an inflammatory response. Fever, aches, headache, nausea, sometimes a rash or itching.
It is a reaction to dying parasites rather than to the medicine itself, and it generally settles within a day or two.
The heavier the infection, the more noticable it tends to be. That makes it more common in people who have had the infection a long time without knowing.
Some parasites can end up in the brain or spinal cord, and that completely changes the situation.
Killing them there triggers inflammation inside a space with no room to swell. That can cause seizures, severe headache or worse.
So this treatment is never something to start on your own if there is any possibility of nervous system involvement. Unexplained seizures, persistent severe headaches or new neurological symptoms need investigating before any treatment, not after.
Parasite groups do not overlap in what they respond to. A stool sample, a urine sample or a blood test identifies what you actually have.
Guessing wastes weeks and leaves the real infection untouched.
And if you have returned from an area where these infections occur with ongoing fever, blood in the urine, persistent abdominal pain, unexplained weight loss or a long-running cough — say where you have been. Travel history is the single most useful piece of information in these cases.
Most of it passes within a day or two. Severe headache, seizures, visual changes or confusion need urgent attention.
Treatment is short, so the usual question is simply having the right course in hand.
